**March 29, 2018 Update**
The survey is now open. Completed surveys are due April 23, 2018.
If you received a letter with a username and password for our online survey tool, you can access the tool at this link:
Online Survey Tool. Please repeat the online survey tool as many times as you have HCPF-enrolled locations; you may use the same username/password repeatedly to do the survey multiple times.

If you have multiple stores, the survey can also be completed using either of our Excel templates. The multi-tab version may be preferable for small chains who may be manually entering data, and the one-tab version may be preferable for large chains who may have the ability to export data from a reporting system.
